About the Role

As a Nurse Case Manager at All Native Group, you will play a vital role in ensuring seamless care coordination and facilitating access to healthcare services for Native American communities.

You will work closely with clinical teams, external vendors, and healthcare partners to ensure timely and effective patient outcomes.

Your primary responsibilities will include:

  • Managing patient care coordination, including ordering complex durable medical equipment and managing care for un-resourced patients.
  • Processing referrals and overseeing durable medical equipment orders.
  • Tracking care processes and ensuring seamless communication with clinical teams and external vendors.
  • Contributing to the development of care plans and revising them based on patient progress and provider input.
  • Maintaining electronic patient records and assisting in consultations with healthcare providers.
  • Screening patients for alternate resources and educating them on their care plans and healthcare options.
  • Coordinating medical appointments and scheduling necessary therapies and services.
  • Conducting follow-ups to verify referrals and ensuring adequate documentation is in place for patient care.
