**About the Role**
We are seeking a skilled and compassionate Case Manager to join our team at Christus Health. As a Case Manager, you will play a vital role in ensuring that patients receive high-quality care throughout their healthcare journey.
You will work closely with patients, families, physicians, and the interdisciplinary team to identify, initiate, and manage optimal patient flow/throughput. Your primary goal will be to enhance continuity of care, smooth and safe transitions, patient satisfaction, patient safety, and length of stay management.
**Key Qualifications**
- Graduate of an accredited school of nursing or Master's Degree in Social Work
- Certification/licensure as a Registered Nurse (R.N.) in New Mexico or Current licensure as an L.M.S.W./L.I.S.W./L.C.S.W. in New Mexico
- Case Management certification (highly desirable)
- Strong interpersonal skills, critical thinking/problem-solving skills, and knowledge of in-house and external resources
