The University of Maryland Charles Regional Medical Center seeks a highly skilled and compassionate Senior Complex Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in promoting optimal patient outcomes across the continuum of care.
In this dynamic position, you will collaborate with members of the healthcare team, community resources, and patients/patient families to address complex social dynamics, legal involvement, and connections with DSS/DDA. Your expertise in navigating barriers and providing intensive care coordination will make a significant difference in the lives of our patients.
We are looking for a professional with a Master's degree in social work and current licensure in the State of Maryland, as well as LCSW certification. A minimum of 3 years of relevant clinical hospital experience is required. If you are a Registered Nurse, a Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN) is preferred, along with certification in Case Management.
