Key Responsibilities- Develop and implement effective care plans that address complex social dynamics, legal involvement, and connection with DSS/DDA.
- Foster strong relationships with patients, families, and healthcare teams to ensure seamless transitions and optimal outcomes.
- Assist patients in navigating barriers to safe disposition, providing education and support as needed.
- Provide educational training on Joint Commission standards of care, promoting compliance metrics related to assessment, discharge appeals, freedom of choice, and abuse/neglect.
- Mastery's degree in social work or nursing with current licensure in the State of Maryland; LCSW certification required for social work graduates, while case management certification is required for nursing graduates.
- Minimum of three years of relevant clinical hospital experience.
- Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN) preferred for Registered Nurses.
