About the Role
The Patient Care Specialist works under the supervision of the Clinical Manager or designated Manager, providing basic patient care activities necessary in caring for the personal needs and comfort of terminally ill patients in various settings.
This role requires collaboration with the interdisciplinary team to provide quality patient care, including weekend and holiday work. Key responsibilities include:
- Providing personal care, including light housekeeping and meal preparation, as directed in the patient's care plan.
- Demonstrating knowledge of standard precautions and ability to perform basic infection control procedures.
- Assisting patients in performing necessary activities of daily living.
- Supporting patients with ambulation and exercise.
- Assisting patients with medications that are ordinarily self-administrated.
- Maintaining patient safety by using proper body mechanics.
Qualifications
To be successful in this role, you will need:
- A high school diploma or GED equivalent.
- Six months combined experience in home care or hospital, and long term care.
- Current licensure as a Certified Nursing Assistant (CNA) in the state of Florida.
- A valid State of Florida Driver's license with a driving record acceptable to Community Hospice insurance carrier; reliable transportation.
