Key Responsibilities:
- Collaborate with primary care teams and behavioral health clinicians to coordinate patient care
- Conduct care coordination activities, behavioral health maintenance, and link patients with community resources
- Utilize telephonic communication to facilitate patient engagement and process optimization
Requirements:
- High school diploma or equivalent required; associate's degree or higher preferred
- Experience in front and/or back office medical and/or behavioral health setting essential
- Strong communication skills, medical terminology knowledge, and problem management strategies crucial
