About the Role

Thegradcafe is seeking a highly skilled Patient Flow Coordinator to join our team. In this role, you will work collaboratively with patients, families, physicians, and interdisciplinary teams to ensure seamless transitions through the continuum of care.

Key Responsibilities
  • Identify and manage optimal patient flow/throughput to enhance continuity of care and smooth transitions.
  • Care coordination and discharge planning are critical components of this role.
  • Fulfill payer-based requests regarding individual members, where appropriate.
Requirements

To be successful in this role, you must have:

  • A Graduate degree from an accredited school of nursing (BSN preferred) or Masters Degree in Social Work (MSW).
  • Licensure as a Registered Nurse in New Mexico or current licensure as an LMSW/LISW/LCSW in New Mexico.
  • Case Management certification is highly desirable.
  • At least two years' experience in a Health Care setting, preferably in an Acute Care Setting.
