Job Title Direct Support Professional - #3005 Location Oxley, AR Schedule Monday – Friday, 8:00 am – 12:00 pm (Part‑time) Pay $12.35 per hour Summary The Direct Support Professional works on an interdisciplinary team that delivers superior quality of service and provides appropriate activities in a safe and healthy environment within the home and community setting. Essential Duties & Responsibilities Assists clients in basic living skills and daily personal tasks, including showers, dressing, housekeeping, grocery shopping, meal preparation and financial budgeting. Provides transportation to and from activities and appointments as needed; a valid Arkansas driver’s license is required. Makes and maintains clear communication via facility logbook and staff meetings to share client needs and progress toward their plan of care by the end of each shift. Accurately completes all required FCC documentation (behavioral reports, incident reports, compliance forms, personal care worksheets, requisitions, maintenance forms, progress reports, attendance logs, and others as deemed necessary). Documents and maintains records for FTA vehicles, including mileage/billing forms in compliance with Medicaid guidelines. Reports incidents or emergencies immediately to a supervisor or delegated authority; this is a safety‑sensitive role. Assists in the formulation, management, revision, and follow‑through of clients’ plans of care to ensure treatment is executed and safety is prioritized. Follows medication‑management guidelines and assists with monitoring and documenting medication procedures. Organizes and conducts recreational activities for clients. Acts as a role model through appropriate physical appearance, dress code adherence, and professional behavior. Treats clients and their families with dignity and respect at all times, representing FCC’s mission. Assists in maintaining FCC facilities and grounds, keeping them clean, orderly, and functioning. Maintains FCC vehicles, ensuring cleanliness and preventive maintenance. Attends required training and continuing education. Provides proof of liability insurance and keeps it current. May undergo periodic driving tests and demonstrate proficiency in operating passenger vans; may also use personal vehicles with proper coverage. Maintains confidentiality of all information related to FCC, Senior Center Programs, and clients in compliance with FCC’s non‑disclosure and HIPAA policies. Qualifications Education & Experience High school diploma or GED; or minimum two (2) years’ experience working with developmentally disabled individuals. Must possess a valid driver’s license. Language Skills Ability to read, comprehend, and write simple correspondence. Ability to present information in one‑on‑one and small group settings. Mathematical Skills Proficiency in addition, subtraction, multiplication, division, and interpretation of basic statistics such as rates, ratios, and percentages. Reasoning Ability Application of common sense to follow written, oral, diagram, or scheduled instructions. Physical Demands Regular hand‑use, standing, walking, reaching, and occasional lifting of up to 100 pounds; requires good vision, including color vision and depth perception. Quality Assurance & Compliance Follow Code of Conduct and Compliance Investigation Procedures. Maintain daily progress notes on consumers as required by funding sources. Ensure weekly, monthly, quarterly, and yearly record updates. Work Environment Moderate noise level; safety‑sensitive function requiring background checks, drug screening, a clean driving record, a valid Arkansas driver’s license, reliable transportation, and state‑minimum liability insurance.
