We have an exciting opportunity for two full-time Care Managers to join our Out of Hospital Care (OHC) program in Bella Vista on maximum-term contracts, with potential for extension.

About the roleAs a Care Manager, you'll work with clients and stakeholders to assess, plan and manage services that enable people to safely return home from hospital. You'll ensure clients have the right supports in place for independence, choice and control, while also promoting the benefits of Care Connect's wider services. These roles are based out of our Bella Vista office: 1 covers the Metro Sydney area & the other covers SE Sydney. Depending on location this can be a hybrid or fully remote role, with requirements to attend the Bella Vista office on occasion for training or meetings.
In this role, you will:
Review referral and intake information, completing client assessments (face-to-face or via phone) within agreed timeframes and KPIsDevelop person-centred care plans – identify needs, explain rights & responsibilities, and set client expectations collaborativelyBuild strong referral networks and connect clients with local resources aligned to their care plans and budgetsProvide service excellence across all stages of the client journeyAbout You…

What you'll bring:
Strong experience in case management or client managementDiploma or degree in Health/Allied Health or related fields OR 6 years + experienceStrong communication, advocacy and interpersonal skillsProven time management, prioritisation and budget management skillsCurrent Australian driver's licence and reliable vehicleProficiency in Microsoft Office and willingness to learn client database systemsImmunisation record in line with NSW Health policy (including COVID-19), or willingness to obtain prior to employmentTo be considered for this role you must have full Australian working rights, current Police & Working With Children Checks – we can arrange police checks for successful candidates. Our process also includes psychometric assessment.
