**Key Responsibilities**

  • Collaborate with members of the healthcare team, community resources, and patients/patient families to promote optimal patient outcomes across the continuum of care.
  • Maintain a smaller caseload that requires increased attention and intensive care coordination due to complex social dynamics, legal involvement, and connections with DSS/DDA.
  • Assist patients/patient families in navigating through barriers that may be preventing their safe disposition.
  • Provide staff education on Joint Commission standards of care.
