At Christus Health, we are committed to delivering high-quality, person-centered care to our patients. As a Population Health Specialist, you will play a critical role in promoting optimal care outcomes by coordinating services and resources across our health system.
About the Role:
This position requires a registered nurse with a Bachelor's degree in Nursing, preferred. The ideal candidate will possess considerable knowledge of healthcare facilities, policies, and issues, as well as community patient care services.
Key Responsibilities:
- Develop and implement care coordination plans to improve patient outcomes.
- Collaborate with interdisciplinary teams to coordinate patient care.
- Provide education and support to patients and families to promote healthy behaviors and self-management.
- Work with clinical leaders to develop and implement evidence-based practice guidelines.
Requirements:
- Minimum of two years of experience in a healthcare setting.
- Related experience with quality, care coordination, and population health preferred.
