As a Healthcare Navigator, you will play a critical role in providing care coordination services to patients, ensuring optimal person-centered care that supports and empowers individuals, respects individual choices, and meets healthcare needs.
About the Position:
This position requires a registered nurse with a Bachelor's degree in Nursing, preferred. The ideal candidate will possess considerable knowledge of healthcare facilities, policies, and issues, as well as community patient care services.
Key Responsibilities:
- Provide care coordination services to patients based on person-centered acuity.
- Coordinate patient care in hospital or clinic settings.
- Contribute to a cooperative and accountable working relationship with other members of the care coordination system staff.
- Work with clinical leaders in the development and implementation of protocols.
Requirements:
- Minimum of two years of experience in a healthcare setting.
- Related experience with quality, care coordination, and population health preferred.
